Pricing and Rates:
All pricing will be determined as per the National Disability Insurance Scheme price guide (subject to rate changed by the National Disability Insurance Scheme).
Transport:
If any support sessions require transport whether in a worker’s car or hire of a bus, I agree to a fee of $1 per kilometre travelled to be charged or the amount of the hire of the bus divided by number of participants. If pick up and/or drop off is required, it will be charged at a 1:1 rate from the time pick up occurs.
Activity:
I agree that any activity costs during support of my child will be paid for by myself.
Liability:
I hereby release the provider against all liability (including liability for their negligence or the negligence of others) for all injury, loss or damage arising out of, or connected with, mine/my child’s participation with the provider.
Personal Property:
I agree that any personal property brought with the participant to any support session that is lost, stolen or damaged, is at the owner’s risk and discretion and the provider holds no responsibility.
I agree that any damages to personal property of the provider, caused by the participant, will be reimbursed in total by the parent/guardian/primary contact of the participant.
Notice Periods:
I agree to provide 7 days notice if I wish to cancel support. If I fail to do so, 100% of the of the full amount will be charged. (Subject to NDIS change)
I will notify the provider of any important changes. i.e. moving house, change of goals or session changes.
Pricing:
Participants plans will be invoiced in accordance with the NDIS price guide. Please specify whether the participant is 1:1 or 1:2 ratio.
